Home-visit request form

The home-visit request form collects your name, email, optional phone number, optional Calgary neighbourhood and the visit type you would like to book. Your submission is sent to Continuum Home Physiotherapy by email so that we can reply and arrange a visit.

The form is protected by Cloudflare Turnstile, which checks automated submissions without collecting personal information about you. Form submissions are sent through Cloudflare to deliver your message to our inbox. Cloudflare acts as our agent when processing your request.

We retain form submissions for as long as needed to arrange your visit and maintain our records, after which they are deleted.

We take reasonable administrative, technical, and physical measures to protect personal and health information from unauthorized access, use, disclosure, loss, or misuse. Information is retained only as long as reasonably necessary to fulfill its intended purpose and applicable legal or professional requirements.
